Does it send the email itself?

Listmonk composes and queues, then hands off to an SMTP relay. Connect Brevo, Amazon SES, Postmark, or whatever you already use.

Can I import an existing list?

Yes, by CSV, and it handles large imports without falling over.

Is my deliverability my problem now?

Partly. You control the sending domain and its reputation, which is more work and more upside than sharing a pool with strangers.
